Is your business a corporation, LLC, partnership or sole proprietorship? In which state is it formally organized? Who are the principal owners of your business, and what are their shares of ownership?
COMPANY HISTORY TO DATE
Is this a new or existing business? For a new business, what inspired its startup? For an existing business, when and how was the company founded? Provide a brief summary of the company’s history.
FACILITIES
Does the business have an existing place of business? Describe the present circumstances and future needs. Will facilities be leased or owned? Are the terms favorable? What adjustments will be made as the business grows?
